Meet CCB PCN – or Chippenham, Corsham and Box Primary Care Network

 

PCN stands for Primary Care Network and is a way of looking at how we work as a collective. Our PCN is The Chippenham, Corsham and Box Primary Care Network, or CCB PCN for short.

Our PCN includes Hathaway Medical Centre, Rowden Surgery, The Lodge Surgery, The Porch Surgery and Box Surgery. It does not mean all of the practices are the same or that we are mixing, however it means we are trying to work better together! We are still continuing our own way of working and nothing is changing regarding your Registered Doctor. It means specialist clinicians are working together for our practices as one. They may be working from any of the above surgery’s.

 

Does this mean I’m registered with another surgery now?

 

You’re still part of the Hathaway family! Your surgery and your doctor will stay the same. You won’t be seen at another surgery unless it is in your best interest. You might be seen by a Physiotherapist in another surgery or seen in our “Hot Hub” but you will continue to be registered at Hathaway.

Who can access my medical information?

 

If you have not opted out of Shared administration our primary care staff are able to access your medical records. However, confidentiality is our top priority. This does not mean we are sharing your information with other organisations, it just means this access will be available for authorised staff working within our surgeries.
