Sick Notes

Hospital care sick notesIf you have received inpatient or outpatient care from the Royal United Hospital, and you feel you need a sick note, however they did not provide you with one, please see this page- https://www.ruh.nhs.uk/patients/fit_notes/index.asp which explains how you can get in contact with the correct department to request a sick note to cover your hospital care.  Please do not contact the surgery for this.

Under 7 days:

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification. You can also fill out this form as a self certification linked below

After 7 days

if you are still unwell and feel you are not fit for work, You are required to make an appointment with a GP. The GP can provide a ‘doctors note’ called a sick note, or Med3 for your to give to your employer.

Backdating sick notes

Sick notes can be backdated at the doctors discretion, this means you do not have to have an emergency appointment for this. Sick notes cannot be future dated.

Requesting duplicate Sick notes

Should your employer request a duplicate signed sicknote, a charge of £10 will be administered for this.
