You can do this by filling in a form yourself when you return to work. This is called self-certification. You can also fill out this form as a self certification linked below

When a sicknote is not required
Do not ask your GP for a sick note if you are off work because of COVID-19. Instead you can get an isolation note to send to your employer as proof you need to stay at home.
Sicknotes can be backdated at the doctors discretion, this means you do not have to have an emergency appointment for this. Sicknotes cannot be future dated.
Requesting duplicate Sicknotes
Should your employer request a duplicate signed sicknote, a charge of £10 will be administered for this.
