This boundary is for new patients who wish to see if they live within our catchment area.
If you are an existing patient who has moved out of this area, please re-register with a surgery within your postcode.
Due to the increasing population in Chippenham, we are writing to patients registered with us who live outside of our catchment area, to ask that you re-register in the area that you live.
We also need to highlight that if you are on our outer boundary and need more home based care, you will be asked to re-register with a GP surgery closer to your postcode. This can be for many reasons, for example; other agencies may be needed in your care who are postcode based, or home visits are not possible due to the distance the GP would have to travel. Chippenham agencies cover Chippenham, most do not cover Calne, Melksham, Trowbridge etc. and therefore cannot provide the care needed.
There is a current system wide issue resulting in the inability to use practice boundary maps- please bear with us while the NHS catchment team work to resolve the problem.
