Sick Notes

Did you know:

You can now request sick notes via the Airmid App. Its a more convenient way of requesting a further sicknote and saves phoning the surgery. To learn how, go to the Airmid tutorials section on this page

Under 7 days:

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification. You can also fill out this form as a self certification linked below

After 7 days

if you are still unwell and feel you are not fit for work, You are required to make an appointment with a GP. The GP can provide a ‘doctors note’ called a sick note, or Med3 for your to give to your employer.

When a sicknote is not required

Do not ask your GP for a sick note if you are off work because of COVID-19. Instead you can get an isolation note to send to your employer as proof you need to stay at home.

Backdating sicknotes

Sicknotes can be backdated at the doctors discretion, this means you do not have to have an emergency appointment for this. Sicknotes cannot be future dated.

Requesting duplicate Sicknotes

Should your employer request a duplicate signed sicknote, a charge of £10 will be administered for this.
