Are you looking after or providing support for a relative friend or neighbour? Are you being helped or supported by a relative friend or neighbour?

Your GP needs to know so you can be offered the right information, support and access to services. GP surgeries have to compile information for the government about the numbers of carers attached to the surgery so your information will help them to do this.

Please complete the form below and return it to the surgery as soon as possible.

There is more information about help with day-to-day living because of illness or disability on the NHS website.

Often carers are unaware of the help/services available from Carers Support Wiltshire (0800 181 4118). You can also access their website which has been developed to represent all the Carers Support Agencies in Wiltshire:

Carers Support Wiltshire runs the following –

There is the carer’s cafe in The Angel on the third Monday of the month, 10:30-12 with free coffee and cakes!

The Creative Carers group meets on the 1st Friday of the month at St Peters church hall, 11-2.This will begin again in April 2019. All carers very welcome.
