An exciting opportunity has arisen in Chippenham, Corsham and Box Primary Care Network for a Project Manager to work closely and support the Primary Care Network board. We are looking for a part time project manager to join our growing team working across our network of 5 GP surgeries working 3 days per week.
Salary £30,000-£40,000 pro rata (depending on experience)
Formed in 2019, Chippenham, Corsham and Box PCN is an enthusiastic, dynamic, and friendly PCN made up of 5 GP surgeries, serving 54,000 patients. We are constantly striving to improve patient pathways and health care outcomes and we are developing innovative ways of working more closely together. As our PCN grows, we will be employing more professionals listed in the PCN Additional Roles Reimbursement Scheme in the coming months and years.
The PCN Project Manager will be responsible for managing all aspects of our Primary Care Network Performance to include the Investment and Impact Fund (IIF) as well as meeting the requirements of the PCN DES Contract, NHS Long Term Plan, Five-year framework for GP contract reform and the Digital, Data and Technology Vision.
The role will be responsible for managing all data collection systems and development of systems and processes for managing PCN performance information in the most effective and efficient way.
Communication is key in this role.
