We care for patients in 4 care homes across Chippenham, these homes receive weekly visits from a GP to manage general clinical needs, and we are on hand for anything urgent that may also be needed.
We work closely with the care staff in the homes and have developed a good rapport with all of them.
We ensure that eligible care home residents are identified and offered seasonal and routine vaccinations in line with national recommendations and ask that consent forms are filled in by staff or family members when patients join the home.
Should you have any queries about your family member in a home, please first discuss this with the home staff, they will then raise it with a GP should it be necessary.
