Best Practice
How to get the best from Hathaway Medical Partnership
Our aim is to help all of our patients meet their health needs. The purpose of this sheet is to help you identify the best way to contact us and access our services.
Patients are no longer registered with any one doctor; you will be registered with the partnership. However, continuity of care is an important part of your healthcare and we ask that you keep seeing the same doctor for your ongoing problems. Staff will direct you to the doctor who has seen you before and will be able to give you details of all appointments that you have had in the recent past.
Continuity
Wherever possible it is always in your interest to see the same Doctor for any episode of care.
In order to meet the rapid increase in local demand and expectations, the Health Service has developed different systems. This includes:
- NHS Direct ~ Telephone 0845 46 47
- Developed roles for local Pharmacists
- Developed roles for Nurses and other Health Professionals
- Developed roles for Health Care Assistants into the surgery
- Developed roles for Reception and Admin Staff
Whilst the Doctors have overall responsibility for the services we deliver they do not have the time to do everything for you.
To get the best from us it is essential that you take guidance from our staff. Patients generally ask for an appointment with a doctor, yet may be seen a lot quicker by one of our specially trained nurses, or sometimes we can help you without you having to come into the surgery through a telephone consultation or by passing on details for an ongoing sick note etc. Sometimes, given the correct information, staff will identify that blood tests may be needed before seeing the doctor. Prescription requests can be posted or faxed to us or submitted through our website. Please note that we do not take telephone requests for repeat medication.
Use our Prescription Request Form.
Remember
Practice Nurses, Health Care Assistants and administrative staff can often resolve many problems that you may have without you having to come in to see the Doctor.
